ACHR COVID-19 Survey – Alabama Council on Human Relations, Inc

Please read the IMPORTANT INFORMATION concerning the opening of our Early/Head Start Centers and complete the survey below:

When the ACHR classrooms reopen and visits in the home resume, we will follow CDC recommendations, as well as HS guidelines and DHR standards. We will have smaller classes (no more than 10 children per class). Children will no longer serve themselves during meals; teachers will serve meals in the classrooms. Children will no longer brush their teeth while at the centers.

We will continue to educate and help children and staff to use proper handwashing procedures. We will implement daily thorough cleaning procedures. Everyone in our centers (staff, children, and parents) will have their temperature taken before entering our centers, or buses. Temperatures will be taken prior to home visits. Adults (including classroom staff and home visitors) and older children (6 years +) will be required to wear masks while in our centers.

***Priority will be given to children going to kindergarten in 2021, children with disabilities, children with parents that need services to work or attend school, and other special circumstances.

A. Yes, my family wears masks every time we are in public

2. Since our centers have been closed, have you and your family been social distancing? *

A. Yes, we have only been with immediate family and gone into public places only for essentials only
B. Yes, we have been at home with immediate family but we have not been able to social distance at work

3. Since the state restrictions have been lifted some, do you and your children go into public places more? *

A. We are continuing to stay home as much as possible
B. We have started going out more but we are still practicing safe social distance and CDC recommendations

5. Considering what you have read about our new practices in the paragraphs at the top of this survey, when we reopen our Early/Head Start centers will you: *

A. Send your child, to the center as normal
B. Teach your child at home with information, resources, and lessons that can be done at home, and support provided by ACHR

6. If your child falls in the priority for the first group who return to the Head Start center but there are too many children in the group size of 10 in a HS classroom which of these would you prefer? *

A. In Center classes offered Monday and Tuesday
B. In Center classes offered Wednesday and Thursday
C. In Center classes offered Monday through Thursday every other week
D. Keep my child at home and work with the classroom staff by phone or video at times scheduled on Fridays to help my child learn until the center can fully open.

7. If you live further than 2 miles from the center, will your child need bus transportation? *

A. Yes, my child needs bus transportation. I do not have a way to transport my child to school

10. Would you be willing to download (or allow ACHR to download) an educational program onto your tablet? (We will set up a log in for each of your HS children) *

11. Will you commit to working with your child at home (help her/him log on to the early learning network on the tablet and periodically monitor learning activities)? *

12. If you do not have a tablet for your child to use at home, would you be willing to sign out an ACHR tablet (you would be responsible for returning this to us in good condition) for your child’s use while s/he is in our program? *

13. If you borrow an ACHR tablet, but Internet is not available can you go somewhere with Internet capability one time per week to upload more learning activities? *
