(Children registering for Head Start must be 3 years of age by September 2 of this year)
Note: acceptance based on a points system, so children may not be accepted in the order applications were received.
Complete the entire application and return with these items:
- Copy of income documentation for the family (all applicants)
- Documentation of earnings from employment from past year
for all adults in household; must
include each job worked in 2019. May be
- most recent 1040 tax returns (2019)
- check stubs
- W-2
- Other documentation of income may be accepted – please ask
- Documentation of TANF, Child Support, SS and/or SSI statements
- Documentation with child’s birthdate (new applicants only). This can be
- Birth Certificate (preferred – you will need one for child to enroll in kindergarten) OR
- Immunization certificate or other official document with birth date
- Any other official document with child’s birthdate
If accepted also must provide: (extra points given if all of the following documents are turned in with application)
- an up-to-date shot record
- well baby/child check
- documentation of dental exam, cleaning and/or treatment, and
- Medicaid/insurance card

* Children who have not turned 3 years old by center start date or who weigh less than 40 pounds will not be able to ride the HS bus until their birthdate.
